Case Management for Growing Scholars Ages 6–13

Student Solutions provides focused case management for scholars ages 6–13 who may need continued support navigating developmental services, disabilities, providers, school-related resources, and community supports. We help families understand their options, coordinate care, and identify clear next steps.

Our Case Management Approach

A — Assess

We assess the scholar’s current needs, family priorities, existing services, strengths, and areas where additional support may be needed. This helps us understand the full picture before building the plan.

C — Care Planning

We create an individualized care plan based on the scholar’s needs, family priorities, current services, and identified goals. The plan outlines clear next steps and supports to help the family move forward.

R — Referrals

We connect families with appropriate providers, programs, community resources, and other supports that align with the scholar’s needs and care plan. We help families understand their referral options and next steps.

M — Monitoring

We follow progress, maintain communication, identify changing needs, and help families stay connected to the services and supports in their care plan.
