Case Management for Ages 6–13
From BabyNet early intervention, through OIDD early intervention services, and into case management, Student Solutions can support eligible families across multiple stages of childhood. Instead of repeatedly starting over, families may be able to continue with people who already understand where they have been and where they are going.
Birth–3 BabyNet → Ages 3–6 OIDD → Ages 6–13 Case Management
Service Areas: Greenville, Spartanburg and Cherokee Counties.
As your child grows, the program may change. The relationship does not have to.
What can a case manager help me with?
- Provide a tailored list of therapy providers for OT, speech, PT, or ABA
- Connecting with community resources such as Family Connection SC, SC Thrive, local programs, recreation, and social opportunities
- Helping with medical equipment and supplies by coordinating prescriptions and connecting families with DME providers
- specialized referrals such as genetics, or respite resources
- Following up on referrals and services so next steps do not get lost
- Monitoring changing needs as your child grows
- Helping organize the bigger picture when multiple providers, school supports, appointments, and services are involved

A — Assess
We start by understanding your child’s needs, strengths, current services, and your family’s priorities. This helps us see the full picture before deciding what support may be helpful.
C — Care Planning
Together, we develop a care plan based on your child’s needs, your priorities, current services, and identified goals. The plan gives everyone clear next steps.
R — Referrals
We help connect your family with providers, programs, community resources, and other supports that fit your child’s needs and care plan. We also help you understand what happens next.
M — Monitoring
We stay connected, follow progress, and help identify changing needs as your child grows. We can also help keep services, referrals, and next steps moving forward.
