Process Pathway

Therapy Services

  1.  Obtain RX: Parents will need to schedule an appointment with their primary care physician to obtain a prescription or referral for the therapy evaluation.

  2. Provide to Service Coordinator: After obtaining the prescription, parents should provide it to their service coordinator.

  3. Identify Compatible Agencies: The service coordinator will help search for and identify compatible therapy agencies based on the family’s individual needs and preferences. 

  4. Evaluation: An initial evaluation will be scheduled to understand the child’s needs and goals for therapy. Please attend this appointment to minimize starting the wait time over. 

  5. Service Plan Development: Based on the assessment, a personalized therapy plan will be developed in collaboration with the parents.

  6. Ongoing Sessions: Therapy sessions will be scheduled regularly, and progress will be monitored with periodic reviews.

Technology

  1. Obtain RX: Parents will need to obtain a prescription or recommendation for assistive technology from their primary care physician.

  2. Provide to Service Coordinator: Parents should provide any necessary documentation to their service coordinator for the referral process.

  3. Consultation: A consultation will be scheduled to discuss the child’s specific needs and how assistive technology can help.

  4. Evaluation & Recommendation: The service coordinator will assist in completing the paperwork and documentation for assistive technology devices and submit to the appropriate teams for the review process.

Autism Evaluation

  1. Initial Inquiry: Parents can start by contacting our office or filling out an online form to express interest in an autism evaluation.

  2. Referral: A referral from a pediatrician or primary care physician may be requested to initiate the evaluation process.

  3. Scheduling: A  meeting may be conducted to gather preliminary information about the child’s development and behavior. Once the referral is made, the evaluation agency will schedule an appointment for a comprehensive evaluation.

  4. Evaluation & Feedback: The evaluation will be conducted, and parents will receive a detailed report and recommendations based on the findings.

Family Training

  1. Request Training: Parents can initiate the process by contacting our office or their service coordinator via email or express their interest in family training services during their service plan meeting.

  2. Needs Assessment: A needs assessment will be conducted to identify specific areas where the family seeks training or support, ensuring that family training is tailored to your unique needs.

  3. Program Development: Based on the needs assessment, a customized training program will be developed that includes goals, content, and strategies to be covered during the sessions.

  4. Scheduling Sessions: The service coordinator will work with the family to schedule training sessions at mutually convenient times, considering the team’s availability and preferences.

  5. Participating in Sessions: Caregivers will actively participate in the training sessions, engaging with the material and applying strategies discussed to enhance their skills and understanding.

  6. Monitoring Progress: Every six months, progress will be monitored to assess the effectiveness of the training and make any necessary adjustments to the program. Feedback will be gathered from the family to ensure continued relevance and support.

Transition Process

  1. Making a Referral (2 years 6 months): The service coordinator will initiate the transition process by making a referral to the local school district or early childhood special education program at least six months before the child turns three.

  2. Identifying Needs: Conduct a general assessment to determine the child’s developmental strengths and areas for growth. This may involve gathering input from therapists, family members, and other caregivers.

  3. Scheduling the Transition Meeting: The service coordinator will assist in scheduling a transition meeting with the school district. This meeting must take place by 2 years and 9 months to discuss the child’s needs and available services

  4. Participating in the Meeting (by 2 years 9 months): Families can accept or decline to participate in the transition meeting, where they can share insights about their child and discuss the transition to school-based services. This is an opportunity to review evaluation results, schedule additional evaluations and eventually develop an Individualized Education Program (IEP) ”if needed.”

  5. Planning for Transition: Collaborate with school professionals to create a transition plan that outlines goals, services, and supports needed if the child enters the school environment at 3 years of age.

  6. Exiting BabyNet (at 3 years old): Upon the child’s third birthday, families will officially exit BabyNet services and begin accessing pre-school services based on family preferences and decisions.
